A 200mm tire will always measure 200mm across the width of the tire. A 200mm tire would have to be a 201mm to be fatter. Common sizes are 190, 200 240, 300, 360. The series, would be the thickness of the tire. Example a 30 series 200 mm tire would be a 200mm wide tire with no side walls. Most bikes run a 70 series or so.

Not always.... Yes, the standard is just as everyone has pointed out. However, some of our O.E.M tires are larger than the 190mm advertised. I.E. Shows 190/50/17, but measures a true 198mm wide. So, I think what I hear being asked is are there manufactures who also do this with their standard line of tires. I don't know of any personally but that doesn't mean they don't exist...
 
At least with car tires, not all tires are created equal, and the sizes aren't alwaya apples to apples

A 325 Mickey Thompson Drag Radial measures more like 350mm+ across.

I have seen 275 street tires vary like an inch from one brand or product line to another.

I still believe that most tires will adhere to the standard sizing charts, so it wouldn't surprise me if there were no known anomalies.



On a off note, 195/70/17 can be found in a road race configuration..
So, I guess anything is possible.

I know there are differences in size between makes and models of tires. I don't know the answer to the question, but I know that in the truck tire world one company may put out a 33" tire that is virtually identical in size to other 35" tires.
