If you have a good recent annual evaluation that should be enough to justify an annual raise. If not, ask what you can do better or what you should have done better to get a raise. Put it in writing and make sure both of you agree. This is your boss, who I suppose, is someone with work with everyday. If a formal presentation is necessary above and beyond your annuall eval to justify a raise then you have a bigger problem that the raise itself and you should start looking elsewhere. If you can, just walk in with a better offer from another employer. Loyalty works both ways in business and never take it personally, if you are getting the shaft move on.
